The application and supporting materials shall be true, accurate and complete. If there is any falsehood, it shall be disqualified once verified. Specific requirements are as follows:

1. The application must be signed by the legal representative of the enterprise and stamped with the official seal.

2. The certification materials include: a copy of the business license, the financial statements (audited) in the past three years, the R & D investment in the past three years, the market share certification materials in the past three years (such as the report of the third party organization, the certificate of the industry association, etc.), the main technical standards in the past three years, the main intellectual property rights in the past three years, the main quality assurance system in the past three years, etc.

3. The application and supporting materials shall be bound in the order of "Application for Manufacturing Single Champion Demonstration Enterprises" or "Application for Manufacturing Single Champion Products", each of which shall not exceed 50 pages in duplicate.

4. The application form and supporting materials shall be provided in electronic form at the same time and submitted in the form of U disk.
